Here's a possible explaination: Machines depends on the energy/heat drawn from The Matrix for an "optimal" level of survival (the Architect said that the machines were "prepared to accept a certain level of survival") so it's not such a far reach to believe that all Machines are in-fact plugged into The Matrix in some manner. Neo has supreme control within The Matrix, it's not such a far reach to see that
Neo could also have control over things plugged into The Matrix from outside of the Matrix. The Matrix is broadcast through the air, it's not such a far reach to think that all machines could be continuously plugged into it. Furthermore, it's not such a far reach for Neo to be able to tap into underlying air streams from The Matrix without the use of a direct tap. Neo's ability to sense and destroy the machines would there for be a reflection of his ability to sense and manipulate The Matrix. While there is a world outside of The Matrix, The Matrix permeates the real world.
I am in close agreement with your views...if the what they call the real world isn't outside of the Matrix, then why didn't Neo ever realize that before? Why was it right after he made such a pivotal choice that he is able to detect a difference? Also, the sentinels did not freeze in the air as would be expected in the Matrix; they fizzled out electrically. Finally, regarding Smith, it is not too farfetched to imagine his "consciousness" imbedded into a human being in the real world; after all, agents do morph from person to person inside the Matrix. The two worlds are interlinked, one being inside the other. Evidence of this view is the man cutting his own hand to see if it bleeds for real or if he can feel actual pain.
